Online Student Registration
The Online Student Registration process is provided for NEW students who live or will live within the District #58 boundaries.

There are TWO STEPS in the Student Registration Process. Both steps MUST BE COMPLETED before your student is registered.

STEP 1 - Fill out all of the online registration forms carefully, accurately and completely. These include the Student Information, Parent/Guardian Information, Health and Emergency, Special Considerations, Immunization Record, and Background Information forms. Filling all of these forms completely will greatly speed your registration process when you get to Step 2.

Login or Register below when you are ready to begin the enrollment process. Additional instructions are provided that will guide you through the Online Registration forms. (NOTE: Registration records which have not yet been processed by Central Registry can also be updated by logging in below.)


STEP 2 - The second step for the Student Registration Process takes place at one of the Center School District School buildings. Click here for a list of Center District Schools. Step 2 must be completed by the biological/legal parent or guardian. The following documents must be presented to begin Step 2:

  • Verification of Address (Two forms) - Required
    • At least one must be either a:
      1. Valid current real-estate contract (not rental agreement) OR
      2. Current utility bills bearing name and in-district address of parent/legal guardian.
    • One proof can be (1) of the following:
      1. Bank statement with name and in-district address of parent/legal guardian.
      2. Current W-2 with name and in-district address of parent/legal guardian.
      3. Current payroll stub with name and in-district address of parent/legal guardian.
      4. Other documents accepted by the District as proof of residency.
      5. Important: Affidavit must be signed. Understand that the officials of the school district may visit the address given and/or contact the real estate company or rental office to verify residency.
  • Student's Registered Birth Certificate with parent name/s listed - Required
  • Student's Immunization Record - Required
  • Student's Social Security Number (optional)
  • Custody Documentation (required if student does not reside with biological parents) Contact Center School District for specific requirements before beginning Step 2.
  • IEP (required if student has an IEP)
  • Previous records (transcript, withdrawl grades, attendance, *Discipline)
  • * Per Missouri's Safe Schools Act - all new students must present documentation describing all "in" or "out" of school suspensions before the student can be enrolled.

* PLEASE NOTE: Step 2 will NOT begin if any of the required documents are missing.

IMPORTANT: Online Registration Forms are maintained in our system for 30 days. After this time, incomplete registration forms are purged.
