Our community partner, Children's Mercy Hospital, will be conducting FREE athletic physicals here at Center High School on July 11th. At this time, this event is only for our high school scholars. This is NOT a walk-up/walk-in event. All scholars MUST sign-up in advance in order to see a doctor and receive a FREE physical. Slots for physicals are limited so please don't delay, get signed up today!
Below is some important information regarding athletic physicals event.
When: Monday, July 11th,
Time: 1-4pm
Where: Center High School (Aux Gym)
Enter on east side of school, doors across from the grass soccer fields.
Appointments are required. No walk-ins will be accepted day of.
Arrive at the appointment time signed up for.
Have all documentation and signatures completed. Have PRIVIT account up-to-date prior to appointment.
Attached is a pre-participation/physical form, if needed.
Following appointment, upload completed physical to PRIVIT.
